Appendix 4 - Application for payment of salary averaged over a calendar year

I apply for payment of my salary by way of equal monthly amounts under the following conditions:

instead of payment being made in full at the end of a calendar month in respect of any payable service during that month, payment will be made at the end of every calendar month (irrespective of whether it is a month during all or part of which there is a requirement to work). The amounts paid will be calculated by application of the appropriate formula for calculation of average payments;
this agreement may be terminated by either party on issue of at least four weeks' written notice. After such termination and while service continues, salary will be paid in full at the end of a calendar month in respect of any payable service in that month;
if for any reason Civil Service employment ends, any amount advanced under this arrangement in excess of what is due in respect of payable service will immediately become repayable without demand and may be offset against any pay/allowances or other payments due.

If my application is accepted I agree to any deductions that may be made in accordance with the conditions set out above.
